Specialty billing

Wound care medical billing services

Debridement coded by the deepest tissue removed and by surface area, skin substitutes with product codes and wastage rules, and payer criteria that hinge entirely on documented measurements.

Also called: Wound Management · Hyperbaric Medicine. We keep one page per specialty rather than a page per name.

Why wound care billing is different

Wound care is the specialty where documentation is the code. Debridement is selected by the deepest tissue actually removed — skin and subcutaneous, muscle and fascia, or bone — and then by wound surface area, with add-on units for additional area. Selective debridement without instrumentation of viable tissue is a different code family entirely. A note that says 'wound debrided' supports the lowest-paying option available, every time.

Surface area is not optional either. It drives the add-on units on excisional debridement and the application codes for skin substitutes, and payers audit it. If length and width in centimetres are not in the note, the units are not defensible.

Skin substitutes are the highest-value and highest-risk part of the specialty. The application procedure and the graft product are two separate claim lines — a CPT application code by anatomic region and area, and a Q-code for the product by square centimetre — and payers apply strict criteria: a documented failure of conservative care over a defined period, wound measurements at each visit showing inadequate progress, and adequate vascular supply. On top of that, unused product has to be reported as wastage.

Hyperbaric oxygen therapy carries its own approved indication list, its own supervision codes, and session limits that are enforced.

Codes we work with

The families this specialty actually bills, and where each one carries a rule worth knowing.

Family Range Notes
Selective debridement 97597, 97598 Without anaesthesia, by wound surface area
Excisional debridement 11042–11047 By deepest tissue removed, with add-on units per additional 20 sq cm
Skin substitute application 15271–15278 By anatomic region and by wound surface area
Skin substitute products Q4100–Q4300 Product codes billed per square centimetre
Negative pressure wound therapy 97605, 97606, 97607, 97608 By surface area and by disposable or durable system
Compression and strapping 29580, 29581 Unna boot and multi-layer compression
Hyperbaric oxygen therapy 99183, G0277 Physician supervision and facility delivery per 30-minute interval
Vascular assessment 93922–93926, 93965–93971 Supports medical necessity for advanced therapies
Evaluation and management 99202–99215 Modifier 25 where a distinct evaluation accompanies the procedure

Modifiers that matter

And the specific way each one is most often misused in this specialty.

Modifier Use Common error
59 / XS / XU Distinct procedural service or separate anatomic site Missing when multiple wounds at different sites are treated in one visit
25 Separate E/M on the same day as a procedure Omitted, so the evaluation bundles into the debridement
LT / RT Laterality Left off bilateral lower-extremity wounds, producing duplicate denials
JW / JZ Product amount discarded, or no amount discarded Not reported on skin substitutes, which payers now require
KX Medical necessity documentation is on file Required by several policies for continued advanced therapy and often omitted
58 Staged or planned procedure in a global period Missing on a planned repeat application, so the claim denies as included
[VERIFY] Coding specifics on this page must be confirmed by a certified coder before publication (specification §6.2).

Top denial reasons

With the CARC code you will see on the remittance, and what we do to stop each one recurring.

Denial CARC How we prevent it
Wound measurements not documented CO-16 Length, width and depth captured as required fields at every encounter
Conservative care criteria not met before advanced therapy CO-50 Payer criteria checked against the documented four-week course before application
Units exceed the surface area supported CO-151 Add-on units calculated from the recorded area, not estimated
Skin substitute or HBOT not authorised CO-197 Authorisation obtained per product, per application and per session block
Application bundled with debridement performed the same day CO-97 Same-day sequencing and distinct-site modifiers applied on the evidence

Prior authorisation in this specialty

Skin substitutes and hyperbaric oxygen therapy are among the most tightly managed services in outpatient care. Payers generally require a documented four-week course of standard wound care with inadequate healing, serial wound measurements, adequate arterial supply demonstrated by vascular study, and glycaemic control where the wound is diabetic. Authorisation is granted per product, per application count and, for HBOT, per session block. We assemble that package to each payer’s published criteria, obtain the authorisation before the product is opened, and re-authorise before the approved application count runs out.

Where revenue leaks

  • Debridement coded to selective when excisional depth was documented, or to the lowest depth by default
  • Add-on area units never billed on large wounds
  • Skin substitute product Q-codes and wastage omitted, so the practice absorbs the product cost
  • Applications performed past an approved count and denied in full

Questions about wound care billing

How do you choose between selective and excisional debridement?

By the deepest tissue actually removed, as documented. A note that only says the wound was debrided supports the lowest-paying option.

Do you bill skin substitute products separately?

Yes. The application CPT code and the product Q-code are separate lines, and discarded product is reported with modifier JW.

What documentation do you need for advanced therapies?

Serial wound measurements, the four-week conservative care course, vascular assessment and, for diabetic wounds, glycaemic control.

Can you manage HBOT authorisations?

Yes, per approved indication and per session block, with re-authorisation before the block runs out.
